RED-S Warning Signs & Prevention

RED-S is an energy mismatch problem. Clusters matter more than single symptoms: missed periods, stress injuries, constant fatigue, mood changes, and declining performance should trigger action fast.

Start here

RED-S begins when fuel does not cover training plus normal body function

The body pays for underfueling by cutting back on hormone function, bone building, recovery, and immunity.

This is not just about food choices. It is about total energy availability across the week.

Athletes can be high-achieving, lean, and still be running a major deficit.

Bone cost

Low energy availability hits bones quietly and then all at once

Low estrogen and rising stress hormones reduce bone building and increase breakdown.

Stress fractures, repeated bone stress reactions, or early osteopenia are not random bad luck.

The longer RED-S runs unchecked, the harder the recovery path becomes.

Prevention

Programs prevent RED-S by normalizing fueling, not by chasing leanness

Do not skip meals, especially around hard sessions, doubles, travel, or altitude camps.

Watch energy, recovery, and injury patterns with the same seriousness as splits, and route health changes to the athlete's family and physician.

Build team language around strength, readiness, and health instead of weight comments or body surveillance.

when to call
  • Coach red flagsLook for clusters, not isolated bad daysConstant exhaustion, stalled progress, repeat illnesses, or slower recovery between sessions are common first signals.Recurring bone stress injuries or pain in low-risk areas should raise the urgency immediately. An athlete who talks about guilt around eating or body manipulation needs support, not praise for control.
  • Vital signMenstrual function is one of the clearest windows into adequate fuelingA regular cycle is not a performance inconvenience. It is a health signal.Missing three or more cycles, cycles longer than 35 days, or no cycle by age 15 deserve referral. Use the referral chain inside a school for who to tell, and in what order.Amenorrhea is not proof of being fit. It is a warning that the system is under stress.
